Summary

Following a reported near-miss involving a reversing vehicle and a staged pallet, depot management has reiterated floor-walking expectations and introduced a clearer near-miss reporting process. No injuries were reported.

Key point: Reporting a near-miss is non-punitive. The purpose is to identify hazards early and prevent injuries.

Required Floor Rules (Effective Immediately)

  • 01High-Visibility Compliance

    High-vis must be worn on active floors and bays. If you forgot it, retrieve it before entering.

  • 02Safe Reversing Protocol

    One driver. One signaler. One plan. If you cannot see the signaler, stop immediately.

  • 03No “Temporary” Pallet Staging

    Staging areas must remain marked and clear. Unsecured pallets may not be placed in travel paths.

  • 04Keep Clear Zones Clear

    Do not step behind vehicles, even “for a second.” Treat reversing alarms as a hard boundary.

  • 05Near-Miss Reporting

    Report via shift lead or the depot log before end of shift. Include time, location, and what changed.

How to Report a Near-Miss

Use the simplest method available at your site. If in doubt: tell a shift lead and ask for it to be logged. Include: what almost happened, where, and what prevented it.
